JOB DESCRIPTION:   SCHOOL SOCIAL WORKER

Qualifications:     Certified by CT State Dept of Education as a School Social Worker

Reports To:         Director of Special Education

General Description:   The School Social Worker works with students, families and school staff around issues of behavioral, social and emotional concerns.  They maintain a working relationship with collateral and community resources.  They provide a safe environment that promotes social, emotional and behavioral health so students are better able to learn.

DUTIES AND RESPONSIBILITIES:
 
  1.  Assess students who are having social/emotional difficulties and develops an appropriate treatment plan using school and community resources
  2. Provides crisis intervention services for students, their families and staff
  3. Serves as a liaison between the school, home, and community agencies for the purpose of helping students both directly and indirectly with their academic achievement and social and emotional adjustment
  4. Provides community outreach activities to assist students and families in need
  5. Compiles social work notes on students and their families as appropriate
  6. Demonstrates oral and written communication skills in an effective and responsible manner
  7. Provides consultative services to school personnel and to families of students experiencing social, emotional and/or academic problems
  8. Provides developmental and interpersonal skill building activities for students
  9. Initiates, facilitates and/or provides follow-up with Family With Service Needs referrals and DCF inquiries/referrals
  10. Assists school personnel and parents in dealing with attendance concerns
  11. Provides individual and/or group therapy
  12. Attends and participates in educational planning including Planning and Placement Team and Section 504 meetings
  13. Participates in personnel development activities which lead to improved job performance
  14. Participates on the Child Study Team and accepts Case Management assignments
  15. Adheres to the NASW professional code of ethics
  16. Knowledgeable of DSM diagnosis and how it affects student behavior
  17. Possesses a basic knowledge of psychotropic medications and their effects
  18. Performs other duties and responsibilities as directed or assigned
 
PHYSICAL REQUIREMENTS:
Must be physically able to complete successfully PMT training
  
Terms of Employment:  Norwich Teachers’ League Agreement
 
Evaluation:  Performance of this job will be evaluated in accordance with provisions of the Board's policy on evaluation of support services.
