Position Title:  Long Term Substitute School Social Worker 
  
Qualifications: 
  • Must possess or be eligible for Connecticut State Department of Education certification for Social Worker (071). 
  • Degree from an accredidated college or university in Social Work.
 
Reports to: Director of Pupil Services and/or designee
 
Evaluator: Director of Pupil Services and/or designee
 
Compensation: In accordance with the collective bargaining agreement between the association and the Board of Education.
 
Work Year: December 16, 2024-March 28, 2025
 
Responsibilities:   To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  Social Workers are prepared to intervene at the individual and system level, and develop, implement, and evaluate preventive programs. In these efforts, they conduct ecologically valid assessments, intervene to establish and support positive learning environments for children and youth from diverse backgrounds, and to ensure that all have equal access to effective educational and psychological services that promote healthy development.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.  The Social Worker has the following duties listed below and other duties and responsibilities as shall be assigned by the Superintendent or his/her designee:
  • Participates in Student Assistance Team, Planning and Placement Team and 504 meetings for “at-risk” youth and/or students already identified with disabilities. 
  • Works collaboratively with school teams to develop, implement, and monitor the use of scientific, research-based interventions and effective educational approaches for students with social, emotional, and/or behavioral challenges. 
  • Maintains ongoing communication with parents and documents conversations in the form of conference notes, telephone logs, or consultation logs. 
  • Provides individual and small-group counseling interventions for students. 
  • Schedules and coordinates the provision of relevant parent training activities. 
  • Maintains and reports students’ progress toward attainment of IEP goals/objectives in district’s data management system.
  • Maintains logs of all services rendered to students, completes required paperwork, and prepares summary reports as required or requested for educational reporting and/or billing purposes. 
  • Conducts home visits and prepares social work assessments as recommended by the PPT and/or as requested by the Director of Pupil Services. 
  • Assists in the daily functions of the school, including dealing with crises and facilitating conflicts among students. 
  • Facilitates referrals to community-based agencies or medical/psychiatric services for students in need of emergency services, medical evaluation or hospital treatment. 
  • Participates in professional development activities offered by Avon Public Schools in addition to any professional activities outside the school setting needed to maintain certification or licensure. 
  • Consults with outside service providers and representatives from community agencies in an effort to identify unique needs and support students across home, school, and community settings. 
  • Performs other duties as appropriate and assigned by the Director of Pupil Services and/or the Special Education Supervisor.
